2. Not Emergency Medical Services

When physician, nurse practitioner, or other provider services are required, those professionals exercise independent medical judgment and are responsible for their own clinical decisions.
Participation in telehealth services may require separate consent forms and additional terms.
8. Laboratory Services
Some clinical services may require laboratory testing.
Laboratory services are performed by independent laboratories.
Processing times, specimen collection, insurance coverage, and laboratory policies are determined by the laboratory performing the testing.
9. Payment and Insurance
Patients are responsible for all applicable copayments, deductibles, coinsurance, non-covered services, and outstanding balances.
Insurance coverage varies by payer and plan.
Submission of an insurance claim does not guarantee payment.
Payments may be processed through RedSail Pay or other authorized payment processors.
10. Appointment Scheduling
Appointments may be scheduled online, by telephone, or in person.
Appointment availability is not guaranteed.
EviaRx reserves the right to reschedule or cancel appointments when necessary.
Repeated missed appointments may result in limitations on future scheduling.
11. Prescription Transfers and Refills
Prescription transfers, refill requests, and medication availability are subject to applicable law, pharmacist review, inventory availability, insurance requirements, and prescriber authorization when required.
EviaRx cannot guarantee immediate fulfillment of all prescriptions.
12. Retail Products
Retail wellness products sold by EviaRx are intended for lawful personal use.
Except where required by law, all retail product sales are subject to our applicable return policy.
13. User Accounts
Certain Services may require you to create an account or patient portal.
You are responsible for maintaining the confidentiality of your login credentials and for all activity occurring under your account.

EviaRx makes no warranties, express or implied, regarding uninterrupted availability, accuracy, reliability, or fitness for a particular purpose.
